Requirements
First Time Issuance
- Certification of Mobility Impaired License Plate form completed and signed by a physician
- Louisiana Driver's License or State ID
Renewal
(Temporary Designation)
- Certification of Mobility Impaired License Plate form completed and signed by a physician
- Louisiana Driver's License or State ID
Renewal
(Permanent Designation)
- Expired Mobility Impairment Placard
- Expired Mobility Impairment ID
Handicap License Plate
- Mobility Impairment ID
- Current Vehicle Registration
- Proof of Auto Insurance
- If the mobility impaired individual is not the registered owner of the vehicle the plate is being issued to, a Designation of Mobility Impaired License Plate form must be completed and submitted
- If the vehicle has not been registered to the owner yet, bring the documents required to register the vehicle.
